We the undersigned student and parent(s) hereby indicate our
family’s intent to actively save and fundraise so our student can
join the Ocean View High School Entertainment Corps in the
Hawai’i Invitational from April 16 through April 22, 2011. By signing
below we also indicate that we understand the initial deposit of
$100 for said competition will be due in June of 2010, with
installment payments due in September, November and December of 2010,
and the remaining balance due in January of 2011, for an approximate
total of $1500/student. *
